An observation checklist for switch controlled IT activities
This assessent is based on the schedule produced by Tina Detheridge (http://www.widgit.com/). I have altered it so it fits in with the Switch access progression that we have found to be useful in school.

I have also made some more check boxes (f- frequently, s- sometimes, n-never, ?- comments) since I found using the original schedule that it didn't give me enough flexibility in recording and that some of the questions it asked did not fit easily with a straight Yes or No. I have also altered some of the questions to be more recordable to direct questions.
I produced these schemes on an Acorn in Impression Publisher with TableMate. These can be downloaded resized,printed, from Impression. The pages are also in HTML graphic pages which can be viewed and printed out from any platform. We also use a standard assessment form to record physical access that I could put on the web site if it would be useful. I usually have all these incorporated with the switch assessment schedules so they are bundled into one assessment package.
